This article intends to discuss the necessity of the Brazilian public health system (Sistema Único de Saúde - SUS) to formulate and implement policies for populations or specific social groups in inequality conditions. It also aims to contribute to the reflections involving iniquities in the health system and to show some impasses, lacunas, challenges and difficulties for the establishment and/or implementation of the Health and National Politics of Integral Health of the Black Population.
