This work is to evaluate regional referral administrative practices in the State of São Paulo, identifying the main difficulties found in the processes of pactuation with a view to integrality of attention. Five regions (Campinas, Piracicaba, Mogi das Cruzes, São José do Rio Preto and São José dos Campos) were selected in which operating reference instruments and the perception of regional and municipal administrators were analyzed regarding this process. The analysis of the interviews with administrators revealed that the formal mechanisms of referencing were insufficient, as were the instruments to accompany them; there appeared to be greater difficulty in the metropolitan regions to grade the references; the strengthening of negotiating spaces between the administrators was identified as a facilitating factor of the process.
