The article makes a historical building of the Italian National Health System and the subsequent structuring of Regional Systems of Health in that country, which has a unitary system that is regionalized and which has been deploying health federalism since the 1990s. It describes the impact of financial crisis that recently befell Europe and puts the successful experience of Civic Audit as the only possibility to guarantee the right to health in a context of scarce resources.
